Canada's Prime Minister Justin Trudeau announced today that a Canadian Vaccine Passport that can be used for international travel will be available soon. The Government of Canada has worked with the provinces and territories to create a standardized proof of vaccination.
"The Prime Minister, Justin Trudeau, today announced that, in collaboration with provinces and territories, a standardized Canadian COVID-19 proof of vaccination is now available to residents of Newfoundland and Labrador, Northwest Territories, Nova Scotia, Nunavut, Ontario, Quebec, Saskatchewan, and Yukon, and will be available in the other provinces soon. Canada is also engaging with international partners to obtain recognition and acceptance abroad, so the proof can also be used to facilitate travel around the world."

If you are in one of the provinces listed above your passport should already look like this and you'll be good to go. For other provinces such as Alberta, B.C., and Manitoba their proof of vaccinations should be updated to this format very soon. You can check if your province's proof of vaccination is the standardized one via this page on the Government's site.
